Federal Benefits Document Checklist (Chapters 30, 31, 33, 35, 1606, and 1607)

If you are applying for federal benefits please complete the benefits packet below, once completed you will receive a copy for your records and the veterans office will receive one for processing. 

Hazlewood Document Checklist (Veterans, Legacy, and Other Dependents)

If you are applying for Hazlewood benefits as a first-time applicant please complete the first-time applicant packet and submit all the required paperwork. if you are applying as a continuing applicant please complete the continuing applicant packet and submit all the required paperwork.

Semester Benefits Form

The semester benefits form must be completed for each semester you are requesting benefits. All transcripts must be submitted to admissions@3588612.com. Failure to submit all necessary documents will result in a longer processing time. 

All forms are fillable and will automatically be sent to veteransaffairs@3588612.com for processing upon completion. If you forget to attach any documents with your packet, please email them with your Name and T-number in the subject line.
